1. Fill out the form above and attach any documents* you have. 

2. A UAT Admissions Advisor will contact you to schedule your transfer evaluation.


3. Once accepted — enroll to complete your transfer to UAT!

*Documents can be screenshots or information to use as unofficial transcripts that reflect your current student status.
Official transcripts are due within one month after enrollment.
