Current UAT students may use the secure form below to request a change to their existing Enrollment Agreement. Provide as much information as possible so the appropriate UAT team can review your request and determine the next steps.

Submitting this form does not automatically change your Enrollment Agreement or guarantee approval. Except for the specific changes approved through this request, all other terms and conditions of your current Enrollment Agreement will remain unchanged and in full effect.
